He might need treating with Marine cure for saltwater ick?? The spots stand out best under 'blue moon' light - 'twilight' conditions. Check his eyes and head as wellas his body to see if there are any signs of outward parasites. Also, your water params? What do they look like? If it is a constant happening and not just every now and then, it could be from internal parasites.
